If you wish to transfer your health record to another service provider, please ask your new provider to send us a transfer application form signed by you. Alternatively if you would like to have your records transferred to Centenary Medical Centre from your previous Doctor, please ask the reception staff for a Transfer of Medical Records Request Form.
Please advise the Reception Staff at the time of making your appointment, if you require an Interpreter Service during a consultation with the Doctor. This will then be booked through the Telephone Interpreter Service (TIS).
Please note that our Doctors can speak the following languages: Hindi/Urdu, Punjabi, Fijian Hindi, Rajasthani and Gujarati.
This Practice is always happy to receive feedback and suggestions that may improve our services. All suggestions or complaints in writing will receive full consideration. A box is provided at reception for your suggestions. From time to time we may invite patients to complete a survey on their perception of the Centre. These surveys are confidential and help us to improve our service to you.

Our standard appointments are (10 minutes). Long appointments (20 minutes) are required for more complex issues such as procedures, health checks, antenatal care and pap smears, Please advise our reception staff at time of booking if you require a longer consultation.
Although we run on by an appointment system, we always accommodate for emergencies such as chest pain, shortness of breath sick babies or children, pregnancy related problems and bleeding and these situation are triaged by our experienced nurses. This could result in a longer wait time for existing booked patients.
National Home Doctor Service (NHDS) provides our patients with an After Hours, In Home and Bulk Billed doctors visit. Please call 13 SICK or 13 7425 anytime outside our clinics normal opening hours. Our Doctors are provided with a detailed consultation report back to this clinic from the NHDS, to ensure ongoing communication and continuity of care.
Home visits are at the discretion of your doctor.
Our Doctors do not take direct calls from patients. Telephone interruptions to the Doctors are only for emergencies. However, you are able to leave a message with the reception staff who will forward your enquiry to the Doctor. We will endeavour to assist you with your enquiry to the best of our ability.
We bulk bill all patients with a current Medicare Card. Private Consultation fees apply to overseas visitors or Students without a Medicare Card. Certain services provided will incur a fee eg. Procedures, wound care, travel vaccinations, pregnancy test. Enquire at the time of booking if there is a fee.
An appointment is necessary for a referral to a specialist. Legal requirements prevent back dating of referrals so please ensure you have a valid referral before your specialist appointment.
Please inform Reception Staff at the time of making an appointment that you require an immunisation. This will mean the appropriate time with the Doctor and the Nurse will be allocated. Please bring your child’s Health Record to all Childhood Immunisation appointments. Travel Vaccination appointments require a long appointment to allow time for Travel Medicine Consultation.
The practice has a system of recalling patients for significant pathology results and reminders for regular health checks. Our current system involves contacting patients via telephone or mail.
